Respectfully Sheweth, That your Petitioner, hath been inform’d, That the Health Officer, will under a resolve of Your Honorable board, be removed, Your Petitioner therefore begs leave to offer himself a Candidate for that Office, as a Professional Man and one who has Served his Country, as a Physician and Surgeon, during the War, and from his attachment to his native country, having resigned a Genteel living, as a Surgeon in the British Army the 22d of April—1775—and on the 23d of Said Month entered into the American Army Under Genl. Montgomery and continued in the Army and Navy Untill Peace was established. If serving his country, and his Abilities being at least equal to any of the Candidates, Joined with a Professional Knowledge has any Merit—in that line to you as Guardians of his Country, he presumes to Apply for Said Office, and in hopes, Your Petitioner in duty bound will ever Pray &c &c
